Staples High School Rugby Registration 2010 - Each season every player must fill out this form and have it signed by your parents. You will not be permitted to play in any of the games or scimmages at practice, unless the coach is in possession of an actual signed Staples Rugby Club Registration Form which includes a Parental Consent & Waiver. Please download this form, fill it out, have one of your parents sign it and bring to next practice.
